I have a current, valid Flying Fifteen Measurement Certificate for the boat entered and can produce it at the
championships on request. I have third party insurance cover of at least €1.2 million covering racing risks in relation to
the boat entered.
I agree to be bound by the Racing Rules of Sailing 2009–2012, as amended, the Prescriptions of the Irish Sailing
Association, the Rules of the Flying Fifteen Association of Ireland, and the Sailing Instructions for this event, and I have
read and agree to the Disclaimer below.

I wish to enter the above named boat in the Championships and I agree to be bound by the RRS, the Prescriptions of the ISA, the Class Rules (where
applicable) and the Sailing Instructions. I accept that the participation of the above named boat in the Championships is at my sole and exclusive risk
in every respect. By this entry I hereby indemnify the Race Committee, Flying Fifteen Association of Ireland, National Yacht Club, its officers,
trustees, members, servants, agents and sponsors in respect of all claims and demands of whatever nature which may be made upon them in
connection with or howsoever arising from participation or intended participation of the above named boat in the Championships, and I acknowledge
that the Race Committee, Flying Fifteen Association of Ireland, National Yacht Club, its officers, trustees, members, servants, agents and sponsors
accept no responsibility in respect of loss of life, personal injury or loss or damage to property which I or any other person may sustain by
participation or intended participation of the above named boat in the Championships or howsoever arising in connection with the event. I confirm
that I carry adequate valid insurance, of not less than €1.2 million, to cover my liability in connection with my participation in the event.
