Historic Preservation Education Grants FINAL REPORT / EVALUATION Since it is difficult to fill out a form using computer word processing software, IHC does not require project directors to use an evaluation FORM. You may submit the evaluation on plain paper, but please use the following standardized FORMAT. 1. At the top of the sheet, centered, type “FINAL REPORT/EVALUATION”. 2. At the left margin, type the following on consecutive lines: “Grant Number:”, followed by the grant number “Title:”, followed by the project title (all in capital letters) “Sponsor:”, followed by the name of the sponsoring organization “Submitted By:”, followed by the name of the person who wrote the evaluation 3. Skip a line, then type the following: If the project was a live program: “Total project attendance:”, followed by the appropriate data. If there was more than one program, give the dates and attendance figures for each. If the grant was used to print materials, type the following: “Items Printed”, followed by the title of the brochure or other item(s) “Number of Copies Printed”, followed by the appropriate figure 4. Type the following questions, each followed by its answer: Did the project meet original goals and reach intended audiences? What did Historic Preservation Education Grant support help you accomplish? Did our grant make possible something otherwise not possible? Explain briefly. Please share any specific stories about the impact of your project on audience members and/or the community. Any lessons learned? What worked? What would you do differently? [Answers to the above questions may be quoted, unless you stipulate that they should not be. ] 5. How did you acknowledge HPEG support? Attach copies of press releases, printed articles, brochures, or other publicity material generated by the program. If photographs were taken at the program, enclose one or more. 6. Attach a list of presenters or advisors who participated in the project, with their institutional affiliations and exact academic titles. 7. If you wish, you may also attach further comments on the project, individual presenters, the HPEG grant program, etc., as well as additional materials.
