T-Mobile and the Customer hereby enter into a Subscriber Contract, the content of which is formed by the provisions of
this Order of Services, the General Terms and Conditions of T-Mobile Czech Republic a.s. for Business Customers in
force or the General Conditions of T-Mobile Czech Republic a.s. for Consumers in force, depending on the status5) under
which the Customer enters into the Subscriber Contract (the “General Terms and Conditions of T-Mobile”), Terms and
Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data in force, the Frame Contract
specified above, the Price List of Services in force, the Voice line Premium Terms and Conditions and by the conditions
of other activated services. By signing this Order of Services, the Customer confirms having received and carefully read
these documents, and agreeing thereto without reservations. The Customer undertakes to read the terms and conditions
of all services that he/she activates throughout the term of the Subscriber Contract; the terms and conditions of the
activated services shall form integral part of this Subscriber Contract as at the date of activation of the particular service.
The Customer shall be provided with the electronic communications services defined by the Voice line Premium Terms
and Conditions and terms and conditions applicable to the Additional Service, in the extent stipulated by the contracting
parties. In the event the Customer refuses or repeatedly fails to collect the subsidized phone, notebook or accessory or
other device through which he/she may use T-Mobile services (jointly referred to as the “Device”), T-Mobile shall store
such a Device for the Customer for 12/24 months following the conclusion of the Subscriber Contract. Should the
Customer fail to collect the Device within this time limit, the Customer hereby grants its consent to donating the Device to
charity. This shall be without prejudice to the validity and effect of the Subscriber Contract.
In the event that the Customer selects, at any time during the term of the Subscriber Contract, a tariff plan with business
terms and conditions that stipulate automatic extension of the Subscriber Contract after the termination of the agreed
fixed period of the Subscriber Contract, the Customer shall agree with the automatic extension of the Subscriber
Contract. The automatic extension of the Subscriber Contract is regulated by the valid business terms and conditions of
the selected tariff plan.
T-Mobile and the Customer agree on the duration of Subscriber Contracts stipulated in the “Services Settings” which form
integral part hereof.
The Customer undertakes to observe the Minimum Monthly Payment in the amount and for the period specified in the
Services Settings; such period shall start on the day of entering into the Subscriber Contract (and shall be extended by
the period during which the provision of Services is suspended). The amount of the contractual penalty and details
regarding the Minimum Monthly Payment are indicated in the Minimum Monthly Payment Terms and Conditions.
The Customer undertakes to use the Voice line Premium service only at the installation address indicated in the Services
Settings. The Customer shall notify T-Mobile of any changes to the installation address without any delay
The Customer agrees that T-Mobile will process the Customer’s personal or identification data (“Data”) in compliance with
the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data and that

T-Mobile may provide information on the Company ID Number, trade name and place of business for the purposes
of Article 5 of Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data.
The consents provided below are applicable to any and all Subscriber Contracts.
The Customer agrees with the publication of Data for the purposes of the T-Mobile Assistant directory service.
The Customer agrees with the publication of Data in telephone directories and information services of the providers
that provide such services.
The Customer requests that the information that he/she does not wish to be contacted for marketing purposes be
included with the Data in telephone directories.
In accordance with the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and
Location Data, the Customer does not wish that business and marketing information containing offers of T-Mobile’s
products and services be sent or provided to him/her.

Location Data, the Customer does not wish that business and marketing information containing offers of products or
services of other entities than T-Mobile be sent or provided to him/her.

Fill in the number or name of the existing Statements of Services if you want to assign the Access card to the existing Statement of Services.

Complete in case you wish to create a new Statement of Services.

Direct debit from a credit/debit card will be set up after T-Mobile receives an authorization form confirmed by the Subscriber. If you select the
electronic Statement of Services, you cannot use the postal order as the payment method.

An obligatory, four-digit password used for blocking Access Card at the Customer Centre. The password may be common to all Access Cards or
individual for each Access Card.

Define the short code for the given Access Card. Options 112 and 15X cannot be used. For more information on the possible short codes, please

If you are ordering a mobile phone for a special price with your Access Card, fill in the requested type of mobile phone from the current T-Mobile
offer for key accounts. To order accessories or a mobile phone for a non-subsidized price, please contact the Customer Centre – Business
