I am a preservice teacher completing the [insert course title] in the [insert School/Faculty] at The
Queensland University of Technology. As a requirement of this course, I will be completing my field
experience in your child’s class/preparatory class/preschool-kindergarten at [insert] on the following

I am really looking forward to working with your child, the staff, and families in this setting. During
my time here I will be observing and working with the children to further develop my understanding
of how children think, play and learn. Part of my teaching responsibilities may include taking
photographs, recording observations and possibly video or audio recordings of a variety of
experiences that your child may be involved in. I may also collect your child’s work samples- with their
permission, and the permission of the classroom teacher. The purpose of this letter is to inform you
as to how I intend to use and handle this material.
The material will be used to plan meaningful and engaging learning experiences for the children in my
classes, in consultation with the classroom teacher. Additionally, as part of my teacher education
program requirements, I am expected to develop a professional teaching portfolio and I would like to
include a selection of photographs and written documentation in my portfolio containing items that
may include your child (for example, photographs and written observations) or that your child may
have produced (for example, samples of their work). This material will be used for the purpose of
“bringing to life” children’s learning in the classroom.
I will use the material only for educational purposes associated with my teacher education program
(for example, assessment, discussion with my supervising teacher) at QUT and for recording my
experiences in my teaching portfolio which may be shared during prospective employment
interviews. Potential employers will not be permitted to make copies of any of these materials.
Images or material contained in my portfolio or teaching content will not contain reference to your
child’s name.
If you agree to the collection and use of your child’s material in the manner as described above,
please read and complete the consent below and return it to me as soon as possible before [insert

The material I collect will be stored securely and will only be used in accordance with this consent,
except where authorised or required by law. At all times you are able to seek access to the material
or content containing images of your child or withdraw your permission
