Absent​ ​or​ ​Errand​ ​Log
Any​ ​time​ ​spent​ ​outside​ ​of​ ​class​ ​needs​ ​to​ ​be​ ​logged​ ​and​ ​acknowledged​ ​by​ ​the​ ​teacher’s​ ​signature.​ ​The​ ​purpose​ ​of​ ​this​ ​form​ ​is​ ​not​ ​to​ ​penalize,
but​ ​to​ ​record​ ​behavior​ ​to​ ​be​ ​used​ ​in​ ​parent​ ​conferences,​ ​grade​ ​and​ ​behavior​ ​conversations​ ​in​ ​the​ ​future.​ ​ ​The​ ​student​ ​is​ ​responsible​ ​for
updating​ ​this​ ​form​ ​and​ ​responsible​ ​for​ ​all​ ​class​ ​work.​ ​ ​Include​ ​Library,​ ​Nurse,​ ​Office,​ ​absences,​ ​tardiness,​ ​and​ ​going​ ​to​ ​another​ ​teacher.
Restroom​ ​visits​ ​are​ ​logged​ ​on​ ​a​ ​separate​ ​form.
