Chance should be monitored during class work to ensure comprehension of directions, task and task completion.
Modifications:
The following modifications will be offered in all educational environments. The teacher providing instruction is
responsible for implementation with the support of his special education case manager.

1. Chance should be seated near the point of instruction, teachers will frequently check in with him to ensure
comprehension of tasks and task completion.
2. Chance would also benefit from a visual and written schedule to help him follow along in class, and/or a visual and
written graphic organizer for multi-step tasks.
3. Mainstream and special education will collaborate to determine if assignments or tests will need to be modified to his
skills level or if extended time to complete assignments/assessments.
4. Chance will be offered to take tests in a small group setting where test that do not assess reading should be read to
him.
5. Larger academic tasks will be broken down into more manageable chunks.
6. Classroom notes will be offered.
7.
Chance will be able to word processing to complete some assignments.

Chance does not display a need at this time. This area will be reconsidered at his annual IEP or sooner if a need is
identified.

Chance will IMPROVE his communication skills in the area of articulation FROM a level of incorrect articulation of
/s, z, sh, ch/ in conversation TO a level of consistently articulating /s, z, sh, ch/ up to the retell and carryover levels as
MEASURED by completion of objectives and documented by the speech language pathologist (SLP) through data
collection sheets.

Area: Academic Performance
Goal:
Chance will increase his use of learned strategies to help him focus on his math functional skills FROM being able to
understand and solve math problems using whole numbers, estimation, factors, fractions, geometry and measurement,
data and statistics, and problem solving with 78% accuracy, to being able to complete these skills with 90% accuracy
as measured by his special education teacher.
