Directions
The supervisor will meet and hold discussions with each individual employee and provide feedback to evaluate
his/her performance for the review period. Comments/examples/feedback should be entered below along with
the supervisor's assessment of the employee's job-specific knowledge, skills, and competencies. At the end of
the review, the supervisor will be asked to provide an overall performance rating. To be eligible for rewards that
are available for performance through the College's Annual Review Process, the individual must perform the
requirements of his/her job well and display the competencies listed below.
Comments are required for the following fields: Job-Specific Skills and Knowledge, Quality of Work, Significant
Accomplishments/Results Achieved, and Areas for Growth, Development, and Improvement. All ratings are
required.
Please provide examples, feedback and a rating for each feedback area, including an overall performance rating.
Rating scale: 1 = Unsatisfactory, 2 = Improvement Needed, 3 = Meets Expectations, 4 = Exceeds Expectations, 5 =
Outstanding.
