Version Update History
v8.0: Full SRM update including remodelled hazardous events and data refreshed to 30th September 2013.
v8.1: Partial update of suicide and trespass hazardous events to take into account new information about specific incidents (see below). The
model structure and data cut-off point were unchanged from v8.0.
In 2013/14, BTP made more information about railway fatalities available to the industry as a result of the enhanced co-operation effected by the
National Suicide Prevention Strategy Group.
BTP provided additional information dating back to 2009/10, which has enabled the industry to update the classification of a number of fatalities.
Further information on the suicide/trespass data reclassification is available in the Annual Safety Performance Report 2013/14.
Further information on v8.0/v8.1 remodelling will be made available in the Risk Profile Bulletin, which is due to be published in August 2014.
Notes
The table in this document presents the 131 HEs on the mainline railway, separated into HETs, HEMs and HENs.
For each HE, the risk, frequency and average consequences per event are given, together with its potential to result in multiple fatality
consequences.
The frequencies calculated for all HETs relate to the frequencies of all incidents per year, whether or not they lead to an injury.
However, for HEMs and HENs, the predicted HE frequencies relate only to the incidents that lead directly to injury.
Due to the level of data available for these HEs, it has not been possible to quantify the frequency of all incidents and determine the probability
of an injury occurring.
Abbreviations used
MF/SF: To indicate HEs with the potential for multiple (MF) or single (SF) fatalities.
ST1/ST2: To indicate that the injuries from shock/trauma are specifically class 1 (ST1) or class 2 (ST2).
